D124 Kindergarten Registration Begins, School Starts Aug. 23

Evergreen Park Elementary Dist. 124 prepares for start of school year. Kindergarten registration begins by appointment only on Aug. 3.

Children attending in grades K-8 will begin the 2012-2013 school year on Thursday, August 23. Student attendance hours on August 23 are from 8:30 a.m. – 11:30 a.m.

BEFORE/AFTER SCHOOL EXTENDED CARE:

The first day for the Before School-Extended Day Program begins August 23, and August 24 will be the first day for the After School Extended Day Program.

The first day for Early Childhood and Pre-kindergarten Program is August 27.

KINDERGARTEN/NEW STUDENTS:

New Student/Change of Address/Kindergarten registration, will take place by appointment only beginning August 3.  Please call the district office (708-423-0950) to schedule an appointment. 

The following documents are required for New Students/Change of Address/Kindergarten registration:

  • child’s original birth certificate with county seal
  • social security number
  • a ‘student in good standing form’ from your child’s previous school

Additionally, parents/guardians will need:

  • current mortgage papers
  • current homeowner’s insurance policy
  • apartment lease listing all tenants; with an original signed/dated receipt from last month’s rent payment and a certificate of inspection

one of these items:

  • Illinois drivers license or
  • Illinois photo ID
  • two additional proofs of residency from the list below

Note: If your area code does not match the Evergreen Park 708 area code, the phone bill must be provided showing your address.  

RETURNING STUDENTS:

Returning students are also required to complete the residency verification process on the fee payment days listed below. Registration packets for all currently registered students were sent home with students to parents/guardians on June 1. 

The packets contained a fee worksheet and an emergency information/residency verification form. The fee worksheet and emergency information/residency verification form, along with the following acceptable proofs of residency from the list below, must be presented at registration:

Additionally, parents/guardians will need:

one of these items:

  • current mortgage papers
  • current homeowner’s insurance policy
  • apartment lease listing all tenants; with an original signed/dated receipt from last month’s rent payment and a certificate of inspection

one of these items:

  • Illinois drivers license or
  • Illinois photo ID
  • two additional proofs of residency from the list below
  • a current lease  
  • two current forms of proof of residency:
    • one of these items:
      • gas bill
      • electric bill
      • credit card bill
      • cable bill
      • water bill
      • phone bill
      • one of these items:
        • bank statement
        • mortgage statement
        • pay check stub (with address)
        • medicaid statement dated the month of July.  

Payment of school fees or arrangements to pay fees will take place at registration.

Thursday, August 9

9:00 a.m. – 12:00 p.m.

1:30 p.m. – 3:30 p.m.

Friday, August 10

9:00 a.m. – 12:00 p.m.

1:30 p.m. – 3:30 p.m.

Monday, August 13

9:00 a.m. – 12:00 p.m.

1:30 p.m. – 3:30 p.m.

5:00 p.m. – 7:00 p.m.

Tuesday, August 14

9:00 a.m. – 12:00 p.m.

1:30 p.m. – 3:30 p.m.

Please note: Fees will not be accepted by mail.

School information may be picked up in all schools on Thursday, August 16 from 11:30 a.m. to 3 p.m. and 4:30 p.m. to 6 p.m. Students' information will not be made availble until parents/guardians without residency confirmation and a receipt from the Business Office.  Please attend one of the dates listed above prior to August 16 to avoid delays at the students' home school.
